This notice is found on the back of all of our Restraint Application Sheets, Please read: We at Professional Products feel there are only two basic indications for placing a patient in restraints: 1. To prevent him/herfrom injuring themselves, and 2. to prevent him/her from irjuring others. We recommend that anyone who places a patient in any kind of restraint review the following points: 1. Explain to the patient and/or family why the restraint is necessary. 2. Choose an appropriate type of restraint. Canvas, cotton, or felt limb restraints, for example, may be adequate for temporary use on a sedated patient in the OR or Recovery Room, but could cause serious damage if used on a violent patient over a long period of time. Body strength as well as general level of awareness and cooperation are important considerations when choosing a restraint. It is extremely important for you to be fully aware of the correct application and sizing procedure before any restraint is applied to a patient. If your are unsure, seek help from your supervisor. 3. In applying restraints, be careful to maintain proper body alignment, both for comfort and to prevent damage to the skin and muscular skeletal systems. 4. After attaching the restraint device to the patient, fasten it securely to the wheelchair, OR table, gurney, or bed as per your hospitals' specific protocol. 5. Record the reason for applying the restraint in your patient notes. Indicate that you are following physicians’s orders or that you have made the decision and will notify the physician. Specify the type of restraint, when it was applied, and how often patient was checked. 6. It is extremely important to keep a careful watch on patients periodically for obvious signs of danger. You should also note any evidence of circulatory impairment: • Is the fingernail color good? • Does the skin blanch normally? • Is capillary refill normal? • Are hands and feet warm and free of edema? • Are radial and dorsal tibial pulses normal? Any change in the above signs should be recorded in the patient’s notes and the restraints adjusted or the type of restraint changed to prevent further problems. 7. Know your institutions policy on using restraints. Some require a physician’s or supervisor’s order before they can be applied. It is your responsibility to follow your institutions policy to the letter, as long as it does not conflict with good nursing practice.
